Here’s a detailed breakdown of what to expect.

Step One

We start with a free consultation

You can tell us about your or your loved one's living arrangements, finances, and any concerns you already have about eligibility.

We listen, ask questions, and give you a clear picture of what the path forward looks like.

Step Two

We bring you some forms to sign

To begin, we'll have you sign a few forms:

  • A DAR (authorization form allowing us to apply on behalf of the resident)
  • Permission to obtain documents
  • Our service agreement and payment

We can come down to your nursing home or do this through email, whatever’s most convenient for you.

Step Three

We coordinate the clinical screening

Medicaid nursing home coverage also requires a clinical assessment, often referred to as a pre-admission screening (PAS), to confirm the resident meets the medical criteria for nursing-home-level care.

We coordinate with the nursing home on this.

***The timeline depends on the facility and the county, but we track it and follow up so nothing falls behind.

Step Four

We review your financial picture before the county does

This is the most important step that many people skip when they try to do this alone

Before approving coverage, the county takes a deep look at your income, assets, and full five-year financial history to determine Medicaid eligibility.

On our end, we make sure everything is set up correctly before that review happens.

If anything needs to be adjusted: like excess income, past transfers, or account structure….we guide you through it.That can include setting up a Qualified Income Trust if needed, and addressing anything in the financial history that could trigger a penalty.

Step Five

We gather every document the county requires

Medicaid requires a lot of documentation, such as: 60 months (five years) of bank statements from every financial institution, copies of life insurance policies, investment accounts, property records, marital records, and more.

It can involve records going back years, and it has to be done exactly right.

We guide you through every document required, and can obtain many of them directly on your behalf, so you’re not stuck chasing paperwork from places you haven’t dealt with in decades.

Step Six

We submit when the application is 100% ready

Once everything is in place, we prepare and submit the NJ FamilyCare ABD application.
